Complete the SL1 Tab

The user can view and edit the SL1 form in the SL1 tab or in the Wizard via Resume in Wizard button.

To view the SL1 form:

1. Search for a policy from Policy tab.
2. In the Policy Search Results grid, select a policy by clicking the Policy Number hyperlink.
3. If there are multiple transactions within the policy, select the desired transaction from the Transactions grid in the Transactions panel.
4. Select the SL1 tab.

To edit the SL1 form:

1. Complete the "To View the SL1 form" instructions above.
2. Select Edit or Resume in Wizard button.
   1. For Resume in Wizards, please visit Wizards section.
3. The Policy / Binder Number and California Premium will auto-populate if the user enters data on the Policy Details panel.
4. Select the appropriate radio button in the Declaration of Accompanying Documents section.
5. Enter information in the (1) Broker Name and 1(A) Broker License Number fields OR section 1(B) and 1(C).
   1. Alternatively, choose a pre-saved transactor from the dropdown (if applicable).
6. Information populates in sections 2(A), 2(B), and 2(D) on the SL1 form if the Policy Details contain data in the following areas: Primary Insured Name, Address of Insured and Location of Risk.
   1. Edit Address of Insured and Edit of Location of Risk are quick buttons to go back to Policy Details Edit page.
7. Information populates in sections 2(C), 2(E) and 3 on the SL1 form if the Transaction Details contain data in the following areas: Description of risk, Coverage Code, and Insurer Name.
   1. Edit Description of Risk is quick button to go back to Transaction Details Edit page.
8. The Nonadmitted Insurers and Percent of Premium will auto-populate if the user enters data on the Policy Details or Transaction Details panel.
9. The broker name entered in section 1 will auto-populate in Signature of Person Named on Line 1 of section 3 - Placement Description.
10. Enter (MM/DD/YYYY) or select a date for Date of Signature.
11. Check the Electronic Signature checkbox.
12. Click Save.

Hints:

Only blank fields prepopulate with applicable submitted or entered transaction data.

If the user enters 1(A) Broker License Number, then section 1(B) Name of Organization and 1(C) Organization License Number are not required.

If the user enters data in 1(A), fields for 1(B) and 1(C) become unavailable.

If section 1(B) and 1(C) are filled, then section 1(A) is not required. If the user proceeds to enter data into 1(A) and there's already information in 1(B) and 1(C), these fields will clear and become unavailable if there's information remains in 1(A).
